4. 2017 EOP First-Time Freshmen Student Services Agreement

EOP is designed to support your educational goals. We are here to help you succeed academically from your freshmen year through graduation. Please review the requirements below:

General Requirements: Freshmen through Senior Requirements

I agree to maintain full-time enrollment (15 units or more) per semester in order to be eligible for the EOP Grant.

I agree to meet with an EOP Counselor or designee as requested throughout my enrollment at Sacramento State.

If my GPA is below a 2.0 or EOP determines my GPA to be "at risk", I agree to meet with my EOP counselor for advising and academic planning.

I agree to seek tutorial assistance as recommended by my counselor and professors.

I agree to modify my course schedule to comply with EOP program requirements and courses.

As a First Year Student: Fall Semester and Spring Semester

I agree to participate in the EOP New Student Orientation on June 27^th and 28^th.

I agree to participate in the EOP Summer Bridge Academy, June 29^th- August 4^th.

I agree to participate in the EOP Learning Communities Program during the fall semester. Placement in a Learning community will be determined by EOP.

I agree to meet with my EOP counselor once per semester, or as requested by my Counselor.

I agree to enroll and complete the Ethnics Studies 98, a transitional course in the spring semester of my first year.

As a Continuing Student: Sophomore through Senior Year

I agree to attend all other retention services offered by EOP that will promote my academic success.

If my GPA should fall below a 2.00 or EOP determines my GPA to be "at-risk", I will meet with my EOP counselor for advising and academic planning.

If I am academically disqualified from the University, I agree to meet the terms of my reinstatement contract in consultation with an EOP Counselor.

If I remain as an undeclared student, EOP will serve as my major department and I will meet with an EOP counselor at least once a year until I declare a major.

I will seek major advising by a faculty member or the Faculty Student Mentor Program (FSMP) once per semester.

I understand that failure to meet the terms of this agreement may result in a "hold" being placed on my registration or I can be dropped from participation in EOP. If dropped, I understand that I will not be eligible for the EOP Grant.
