If selecting an Event type of "Deaf or Hard of Hearing student enrolled in my course," please provide the course name, course section number and five digit class number in the field. Do NOT disclose student information on this form.
Video Schedule: Please provide a schedule of when the videos submitted for captioning will be shown in class/event. Indicate the names of the videos along with the dates they will be shown. e.g. Video Name: "Maya Lin", Date Presented: February 23, 2011.

Copyright Release Requirements

Copyright Release Requirements:
If you have not obtained copyright release, upon submitting this request form, you must obtain a written and signed copyright release letter from the publisher of the media file. Signed copyright release letters must be submitted to the Instructional Materials Specialist in AIRC 2006, Mail ZIP 6065 before we can begin any captioning work and release video information for posting final captioned and /or transcribed media file to a media server, online course, or other location.

Captioning/Transcription Service Type

What type of Captioning Service do you want to use? For a full description of service types, visit the Captioning Services at Sacramento State web page. Please note that for both services, you should submit requests at least 4 weeks in advance from the date you will need them captioned.

Outside Vendor:
Automatic Sync Technologies AST CaptionSync. This service is free of charge for videos that will need to be accessed by a Deaf or Hard of Hearing student enrolled in a course or for requests that are clearly High Priority in terms of access. All other requests will be charged.
In-House:
DocSoft Audio Mining tool. Service is free of charge and available for academic work related requests.

  • Captioning: $36.50 per video hour
  • Standard Transcription: $80.70 per video hour
  • Rush Transcription: $115.00 per video hour

*All video/audio files we process need to be stored on the campus media server. If you do not have a media server account, request a Media Server Account by completing the Media Server Request Form. See the Video Guidelines for Captioning page to learn what formats to provide your videos in. If your video is not in the proper format, you can visit the Faculty Staff Resource Center project rooms located in AIRC 3012A or 3012B to edit your media.

Submit Video/Audio Files

Please upload your video/audio files to your Media Server Folder. If a media server account cannot be set up in time, please provide your videos on DVD or CD to the IRT Service Desk located at AIRC 2005; include a note to Attention: Cryssel Vera with the media indicating that the videos are part of a captioning request.
